EVALUATION OF JESSNER’S PEEL IN ACNE VULGARIS WITH HYPERPIGMENTATION RESULTING FROM STEROID ABUSE

INTRODUCTION

Steroid abuse: Topical corticosteroids are widely marketed in India and can be readily available on medical store with different brand names. There are several factors responsible for corticosteroid abuse like illiteracy, lack of knowledge, lack of qualified medical personal in rural areas, false believe on quacks and neighbours, uncontrolled marketing by medical store, steroid dependency etc.



Mainly potent & mid potent steroid preparations are responsible for adverse reactions. Patients usually give history of different preparation like Bet- - -ate (N, C, and GM), Pan - - rm, Qu- - -derm, Cl- - - te GM etc. Steroid over use can cause macular hyperpigmentation, steroid induce acne, telangiectasia, localised skin atrophy, hypertrichosis (1).

Jessner's peel solution, formerly known as the Coombe's formula, was pioneered by Dr Max Jessner, a German-American dermatologist. Dr Jessner combined 14% salicylic acid (14%), lactic acid (14%), and resorcinol (14%) in an ethanol base. It is thought to break intracellular bridges between keratinocytes.  Jessner's Peels are typically layered until frosting appears. Once neutralized (cool water wash), the patients experiences redness or a tan coloration to the skin for several days. This is a normal phenomenon. Flaking/peeling starts after 3-4 days of application and is typically finished in 8-10 days at most.
